Medicines And Drugs Used In Disorders Of The Joints
Ibuprofen (Brufen)
This is a milder anti-inflammatory drug causing less irritation of the stomach and other adverse effects. This drug is available in combination with paracetamol as well as dextropropoxyphene. It is widely used to relieve pain and inflammation of the joints and muscles, mild to moderate pain of menstrual cramps, dental surgery, and athletic injuries as sprains and strains. The recommended daily dosage is 1,200 to 1,600 mg to be given in 3 to 4 divided parts.
Precautions
It should be taken after meals.
Those allergic to aspirin, are likely to be allergic to this drug also.
It should not be taken by patients of peptic ulcer, or liver or kidney disease.
It should be used cautiously by patients of asthma. It has been reported to cause bronchoconstriction. It is also not recommended for those suffering from hypertension or heart disease.
If symptoms of allergic reaction occur, stop the drug and consult the doctor.
Naproxan (Naprogyn, Artragen), Naproxan Sodium (Xenobid)
Naproxan is effective and relatively less toxic. Besides that the duration of its action is longer. The drug can be taken in a dose of 125 to 250 mg, two times a day with meals. The adverse effects of this drug are the same as those of ibuprofen.
Mefenamic Acid (Meftal)
Mefenamic acid has properties like those of aspirin and can cause gastro-intestinal side effects including severe diarrhoea in some cases. Mefenamic acid is taken in a dose of 250 to 500 mg, three times a day.
Ketoprofen (Ostofen, Profenid)
This is a newer drug and supposed to be more effective than ibuprofen, but can cause all the side-effects as ibuprofen. The recommended daily dose of ketoprofen is 50mg, 2 to 3 times a day.
Flurbiprofen (Arflur)
This drug is given in a dose of 100 mg, twice a day. This drug is less irritant to the stomach than the drugs described earlier. The most common adverse effect is a headache. In rare cases it may cause skin rash and bone marrow depression.
Ketorolac Tromethamine (Ketorol)
It is more effective when used for short-term (5 days) management of pain of moderate severe intensity. Its initial dose is 30 to 6Q mg IM (intramuscular), to be followed by 15 to 30 mg every 6 hours.
Diclofenac Sodium (Voveran, Relaxyl, Flexyl, Diclomol)
This is a potent anti-inflammatory drug but has a short duration of action. Its sustained release preparation YOYERAN-SR (100 mg) is given once daily. Its adverse effects OCCur in about 20 per cent of the patients in the form of distress in the stomach. In some patients it may cause gastric ulceration.
Piroxicam (TOLDIN, PIROX)
It has a long duration of action and is given in a dose of 20 mg, once daily. The adverse effects are related to the stomach. It may also cause dizziness, headache, ringing in the ears, and skin rashes.
Note: The use of corticosteroids is discussed in the chapter on ‘Corticosteroids’, for nimesulide (Nimegesic) and other COX-2 inhibitors see chapter on ‘Pain Relievers’.
Combination Drugs
Hundreds of combinations of drugs containing pain-relievers, anti-inflammatory drugs, sedatives, and corticosteroids are available in the market. These combinations are always more expensive and apparently less effective than the individual drugs. In our opinion, a patient should take the appropriate individual drug rather than expose himself to the hazards of several drugs together, and at the same time, avoid unnecessary expense.
Local Pain Relieving Preparations
A number of drugs are now available as local gels, creams, ointments and liniments (need rubbing) for relief of sport injuries, sprains and joints paints. These are given in the table given below.

Laughter is the best Medicine-, it is a common saying we have heard for countless years. But is it true?

We laugh for different reasons, what I find humorous may not necessarily make you laugh. But whatever makes us laugh makes us feel good. And for a detectable health reason-

But make a point of seeing a funny movie, or hiring a DVD regularly to lighten the mood. Buy TV series that you find funny and watch them every now and then. But make it a habit; it is the habitual nature of things that bring us down. Changing bad habits to good will also raise the energy and laughter in our lives.
Remember, laughter is the best medicine. Life is fast paced, hectic and chaotic at times. Taking a break and having a laugh with friends or family is one of the best remedies you can have – go ahead just laugh.

The Herbal Standardization Process
Over the past years, recognized world authorities on botanical alternative medicine have defined, and established, specific standards of excellence for herbal extracts. Most importantly, we should standardize for the individual key compounds which have been empirically and scientifically proven to be the most advantageous for the human system.Our standardization process should guarantee a consistent and appropriate level of each plant’s medicinal elements within each of the product formulations we sell.
Standardization Standardization of herbal products is a controversial issue. On one hand, herbalists sometimes feel that highly purified and standardized extracts don’t genuinely represent all the best qualities of herbs and can sometimes lead to safety issues, especially when they are highly concentrated and purified. On the other hand, when herbs are harvested and shipped overseas to faraway places and then made into commercial products such as capsules or tablets, it is very difficult to follow what happens to those herbs along the way. For instance, how long ago were those herbs harvested, how long they have been stored in the warehouse, and what adverse environmental conditions such as excessive heat could have contributed to the degradation in the quality of the herbs.
Modern Testing Today, we have highly sensitive analytical equipment such as high-pressure liquid chromatography (HPLC) to ensure proper identification, levels of active constituents, and purity of the finished product. This can be accomplished without materially altering the internal balance of the original herb. Many modern standardized products today do follow a philosophy that takes the whole plant as the best standard for quality, not isolated and purified individual constituents, though these types of products also are sold. Look at the label, and if you see products where the active constituent is 40 or 50%, even up to 80% of the total weights of the product, then you have a highly purified standardized extract. Need of Standardizations In the global perspective, there is a shift towards the use of medicine of herbal origin. As the dangers and the shortcoming of modern medicine have started getting more apparent, majority of Ayurvedic formulation are prepared from herbs. ?Macro and Microscopic Examination: For Identification of right variety and search of adulterants. ?Foreign Organic Matter: Remove of matter other than source plant to get the drug in pure form. ?Ash Values: It is criteria to judge the identity and purity of crude drug – Total ash, sulfated ash, water soluble ash and acid insoluble ash etc. ?Moisture Content: To check moisture content helps prevent degradation of product. ?Extractive Values: These are indicating the approximate measure of chemical constituents of crude drug. ?Crude Fiber: To determine excessive woody material Criteria for judging purity. ?Qualitative Chemical Evaluation: It covers identification and characterization of crude drug with respect to phytochemical Constituent. ?Chromatographic Examination: Include identification of crude drug based on use of major chemical constituent as marker. ?Qualitative Chemical Evaluation: Criteria to estimate amount the major class of constituents. ?Toxicological Studies: Pesticide residue, potentially toxic elements, and Microbial count approach to minimize their effect in final product.
Physical evaluation: Each monograph contains detailed botanical, macroscopic and microscopic descriptions of the physical characteristics of each plant that can be used to insure both identity and purity.
Microscopic evaluation Full and accurate characterization of plant material requires a combination of physical and chemical tests. Microscopic analyses of plants are invaluable for assuring the identity of the material and as an initial screening test for impurities. Most manufacturers of herbal products lack the quality control personnel to accurately assess plant identity and purity microscopically. Ideally, submitted materials should be in their whole or semi-whole form for microscopic assessments.
Chemical evaluation A chemical method for evaluation covers the isolation, identification and purification. The chemical tests include colour reaction test, these tests help to determine the identity of the drug substance and possible adulteration.
Biological evaluation Pharmacological activity of certain drugs has been applied to evaluate and standardize them. The assays on living animal and on their intact or isolated organs can indicate the strength of the drug or their preparations. All living organism are used, these assays are known as Biological assays or Bioassay.
Analytical Methods Critical to compliance with any monograph standard is the need for appropriate analytical methods for determining identity, quality, and relative potency. There are a plethora of analytical methods available. However, it is often difficult to know which is the most appropriate to use.
Chromatographic Characterization
Chromatography Chromatography is the science which is studies the separation of molecules based on differences in their structure, composition. Chromatographic separations can be carried out using a variety of supports, including immobilized silica on glass plates (thin layer chromatography), very sensitive High Performance Thin Layer Chromatography (HPTLC), volatile gases (gas chromatography), paper (paper chromatography), and liquids which may incorporate hydrophilic, insoluble molecules (liquid chromatography).
Purity Determination Each monograph includes standards of purity and other qualitative assessments which include when appropriate: foreign matter, ash, acid-insoluble ash, moisture content, loss of moisture on drying, and extractives.
Quantitative Analysis The primary goal of the method is to provide validated methods to be used for the quantization of the compound most correlated with pharmacological activity or qualitative markers as determined by the primary pharmacological literature, constituent declaration in product labeling, and a survey of experts.. In this context, validation consists minimally of a two-lab validation using the same procedures, samples, and reference standards. Primary factors for considering a method as appropriate include accuracy of the findings, speed, basic ruggedness, applicability to a large segment of the manufacturing community, and avoidance of the use of toxic reagents and solvents. In an attempt to promote harmonization, primary consideration is given to those methods which are already accepted in official pharmacopoeias. When necessary, comparative tests shall be conducted to determine which of the available method is most appropriate. The validation process minimally includes: standard precision, linearity, sample precision using replicate samples, sample linearity, selectivity , retention times, and limits of detection..
Difference between a herbal extract and standardized herbal extract “Herbal extract” is sometimes also referred to as a tincture, or liquid herbal extract. This is a preparation where a whole herb is steeped in alcohol, water or a combination. A “standardized herbal extract” is a measurable marker substance that is extracted from the herb. This marker may be an active ingredient, or just one that is easily determined, but often, it is a compound that has been used in scientific research.
HPTLC ANALYSIS ON HERBS HPTLC is the most simple separation technique today available to the analyst. HPTLC is a qualitative tool for separation of simple mixtures where speed, low cost and simplicity are required and it is also a tool for quantitative analysis . High-Performance Thin-Layer Chromatography for the analysis of medicinal plants presents the theoretical and technical information needed to perform reliable and reproducible results in order to establish the identity, purity, quality, and stability of raw materials, extracts, and finished botanical products.

APPLICATIONS OF SPECTROFLUORIMETRY ON HERBS When a beam of light is incident on certain substances, they emit visible light or radiations. This phenomenon is known as fluorescence. In fluorescence measurement two wavelengths are involved i.e. the excitation wavelength (?ex) and emission wavelength (?em).The fluorescence phenomenon involves the absorption of excitation radiation by molecule which then loses energy by internal conversion processes, before emitting a photon of radiation at lower energy. The excitation wavelength maximum (?ex) is lower than the wavelength of maximum fluorescence emission (?em) Advantage High Sensitivity Substances that are reasonably fluorescent in the herbals like flavonoids, tannins, steroids, etc. may be determined at concentrations up to 1000 times lower than those required for absorption spectrophotometry.
Selectivity The facility to vary independently the wavelength of excitation and the wavelength of fluorescence allow the analyst to select the optimum combination of wavelength for the analyte and to reduce interference from other fluorescing species in the sample.

EXCELLENCE OF SIDDHA MEDICINE
Siddha system of medicine is one of the oldest medical systems of India, existed separately in early times. The system has flourished very well in India for many centuries. Although this system declined in later years, in the wake of changing mode of life and modern medicine, it continues to sustain its influence on the masses because of its incomparable intrinsic merits.
Siddha system has its own well developed chemistry and its tireless efforts striving in the direction of the development of ALCHEMY has resulted in the genesis of thousands of mineral and metallic preparations. Thus Siddha system is well equipped to combat any disease, even chronic diseases, which baffle and elude the sophisticated modern medical science.
The foundation of countless prescriptions forms the most impressive bulk of Siddha wisdom. But even modern experts trained in Botany and pharmacology, will find it hard to understand the complex prescriptions that are given and extremely difficult to identify the plants and herbs mentioned by Siddhars (the founders of Siddha system of medicine). An enormous variety of widely differing drugs were mixed in Siddha medicine into a kind of arcanav, as a myth, mystery or secret. To be sure, the myth also reflects the integration of Siddha traditions and gives a symbolic representation, expressing ancient thoughts. It is not merely an irresponsible play of fantasy but teaches in pictorial scripts, an attitude towards life.
Unique Siddha medicinal preparations: For a medicine to be effective, the inorganic substances have to be brought to their atomic form. Siddhars developed the knowledge of bringing inorganic substances into atomic and ionic form which is easily absorbed by the system, when ground with herbal juices and put on the fire. The specialized Siddha preparations are -parpam’, -chendooram’, -chunnam’and -mezhagu’. These are metallic and mineral preparations by transmutation processes of converting the metal to ashes and waxy constituency which exerts therapeutic properties and devoid of any metallic traces. These are also the by products noticed during Siddhars Alchemical practices only.
Muppu: Muppu has a distinct place in Siddha medicine. It is the combination of three rare salts which enhances the efficacy of any Siddha medicine. The preparation and ingredients are a closely guarded secret. It is also believed that -muppu’ also engender yogic concentration, a part of Siddha tradition.
Kaya kalpa: This is similar to -Rasayana chikitsa’ of Ayurveda and -gerontology’ of modern medicine. The word -kalpa’ means -able, competent’. When it is connected with -kaya’ or body, the term means competency or ability of the body. While in Ayurveda, rasayana is different from -vajeekarana’, kaya kalpa does not treat vajeekarana or the science of aphrodisiacs separately. When kayakalpa treatment is undertaken, the vigor of body and mind are restored and so there is no need for separate attention to vajeekarana. Siddhars were more concerned about the ultimate goal of living and less with worldly pursuit of sex. However, kayakalpa is very much capable of achieving the ends of both rasayana and vajeekarana.
The details of kayakalpa treatment are- 1. Preservation of vital energy of the body by diverting the internal secretions to the circulation of blood by control of breathing through yoga 2. Conservation of sperm by using it for regeneration. 3. Use of universal salt -muppu’ for rejuvenation. 4. Use of calcined powder prepared from metal and minerals such as mercury, gold, sulphur, mica, copper etc. 5. Use of drugs prepared by certain rare Indian herbs.
-Food is medicine and medicine is food- and -Prevention is better than cure- were the slug lines of Siddhars and their followers. In the past people had the first hand information of the health and toxic effects of various species of flora and they provided the raw materials to the medical man for compilation. Thus Siddha system is a science perfected into an art, coupled with primitive intuitions into nature’s resources and secrets.
A close look at the eating habits of animals and birds reveals they eat food raw and ripe consisting of all the six tastes where as man eats only food stuff of four tastes, leaving bitter and astringent, which are pre-requisites for healthy circulation f blood. Bitterness fights against bacterial infection and astringency maintains the right blood quality and its constituents. Small quantities of agathi grandiflora, bitter gourd, seeds of solanum xanthocarpum, flowers of neem eaten now and then in small quantities, ward of many microbiological and bacterial infections.
It was a common knowledge for many Indian to possess the rare knowledge of various medicinal properties of ingredients in our culinary preparations and thus many an Indian woman trained in the traditional oriental ways of food preparation is an excellent and masterly physician without any master degree-

The Hindu Icons or the Pantheons of various Gods and particular modes of worship with leaves, flowers, milk, butter, honey, ghee, betel and nuts are symbolic and of great significance just as an abacus is used to induce children to learn arithmetic; these observations have been designed to promote for living a healthy life of body, mind and spirit. The practices may seem succinct or stupid to the modern civilization, but they were quite significant and sacred in the past.
The ancient wisdom of our ancestors should not be ridiculed because of our ignorance or non realization of their intrinsic worth. Let me present few examples of these rites and rituals and anyone with faithful, sincere interpretations of facts will come to the conclusion as how the medicine, art, culture, rituals and the way to healthy living with longevity were interwoven during prehistoric period.
While worshipping Lord Shiva, during the chanting and pooja, beal leaves (aegle marmelos) are offered to the Deity and after worship, some of these leaves are eaten by the devotees as -Prasad’ (blessings from God).this intake of beal leaves when followed regularly ensures good health by proper digestion and assimilation of food. It also removes the toxins in the body and is a blood purifier.
While worshipping mother Durga, the Goddess is being invoked with neem leaves and later consumed by the devotees. This is manna from heaven when consumed regularly in small quantities with its essence, as it proves wonderful disinfectant and germicide. Regular intake of this herb in any kind will be a sure elixir of life.
The foremost Hindu Deity Lord Ganesha, first to be propiated before commencing any important human project as per the belief. The Lord is worshipped with a particular type of grass – -Doorva’ (cynodon dactylon), which has been proven scientifically to possess the power of generating electricity in minute degree. When handling it augment one’s personal magnetism. It is of high medicinal value when administered orally. Another instance may seem superstitious but not without truth. Before initiating any auspicious rites, it is customary to make a crude image of Lord Ganesha with a fresh paste of either cow dung or turmeric paste and stick a small tuft of Doorva grass leaves to the person of the Deity. This image is abandoned and left to wither after performing pooja. Normally wet paste of either cow dung or turmeric when left exposed to atmosphere will be infested with bacteria and this does not happen with the sanctimonious Lord Ganesh’s image with the bunch of Doorva grass. It may baffle one’s imagination and one may not believe, but seeing believes!!!
Mango leaves are used in the worship of the Goddess of rivers- Ganga devi. Festoons of mango leave adorn temples and houses during festivals. A tuft of mango leaves dipped in the sacred water after worship Ganga is sprinkled over all the places of the premises and on the devotees. This serves as purificatory process. A decoction of mango leaves taken at regular intervals will cure many bodily illnesses.
The people of -Vishnu cult’ perform pooja of the Lord with -Tulsi leaves’ (ocimum sanctum). After the pooja 3-5 leaves are consumed as -Prasad’- these leaves consumed at regular intervals will build strong muscles and bones. It is also a wonderful prophylactic drug for many ailments. Likewise -Lord Brahma’, the Creator is worshipped with country fig leaves and fruits. The intake of these leaves and fruits will restore the correct blood constituents due to its astringent properties. These few narrations give a glimpse into the astounding possibilities of simple herbal preparations with no intricate knowledge of orthodox Siddha compounds with elaborative preparation processes. With a primary knowledge of hundreds of species in vegetable kingdom and the knowledge of physical body, its physiology and anatomy and simple rules of health and hygiene, any layman can lead a healthy life free from diseases.
Similarly periodic oil-bath promotes real rejuvenation of the physical body, though the modern medical conception put it that the diet we take orally contain oily matter which could suffice for a healthy body. Folds and wrinkles disappear in due course, when taken regular oil bath, giving the person youth, glow and luster. There are many illustrations of Siddhars stating by proper physical exercise and perfect breathing, food habits, evacuation of bowels, many undesirable conditions get eliminated. Siddhars have also propounded many prescriptions for rejuvenating the body and this bulk is called -Kayakalpa’, literally meaning a body like stone. The most intriguing, perplexing and unique phenomenon of Siddha system of medicine is the -Kayakalpa group of herbs’ which is not found in any other systems of medicine in the world. These ensure physiotherapy, psychotherapy, preventive and curative aspects of diseases and pave the way for longevity. There are 108 kayakalpa herbs; many among them in daily use are onion, garlic, ginger, lemon, jequirity and turmeric etc.
On close scrutiny one will struck with much wonder as in how those days where there was not even a microscope, the ancient Siddhars were capable of describing the use of such things rich in vitamins, minerals, proteins, hormones, electrolytes and other nutrients grouped them as disease preventives for promoting longevity.
To conclude these scriptures and sciences embody all that was required for human advancements in the world and a study of these ancient literatures will be really productive of lasting benefits. We may confess that we are still ignorant of many things connected with the ancient thoughts and beliefs and it is our ignorance that belittles their greatness.
The revival of ancient systems, be they in medicine or moral code, requires a faithful interpretation of the views of the originators of these sciences and such a movement will be useful and indeed found necessary since modern discoveries still are incomplete and look forward to the ancient works for the -MISSING LINKS’.
The Siddha system of medicine deserves careful and faithful examination since it has in itself many principles which may go a long way to supply the defects and deficiencies of modern conceptions and that may save Indian society and generally the world from many a besetting peril-..
